Please complete the appropriate enrollment forms for the service(s) you are interested in.  Note that some of these forms are only necessary for initial enrollment, updating your personal information or for a new pet.  Vaccination records are required for all services.

 

Pet Enrollment Form  (Cats & Dogs - All Services)
This is the basic but essential information.  Your name, your pet's name, address, telephone # and all that stuff.  This form is necessary to leave a pet at UT for any reason so we know how to find you and which pet goes with which owner.    You need this form for Dog Daycare, Cage-free Sleepovers, Luxury Loft Boarding, Day Boarding, Kitty Boarding, Grooming/Bathing or Training.

Dog Personality Profile (Dog Daycare)
This form is needed for any dog applying to stay with us in a social setting with other dogs, being Dog Daycare &/or Cage-free Sleepovers.  It gives us a lot of preliminary info about your pet so we can plan the temperament evaluation accordingly.

 

Cat Personality Profile (Cat Boarding)

Dog Daycare & Sleepover Agreement (Dog Daycare & Dog Cage-Free Sleepovers)
This is the agreement that sets out drop off times, pick up times, rates, etcetera for dogs coming for Daycare or Cage-free Sleepovers.

Dog Daycare Package Agreement (Dog Daycare)
If you want to buy a 10-visit package for dog daycare, versus paying each visit, we need this form.  It sets out particulars like that it's good for 3 months, discounts, etcetera.

 

Loft Pooch Personality Profile (Dogs - Loft Boarding)
This form is needed prior to the first visit by a dog that has not been here before and is wanting to stay in one of our Loft Suites.  If your dog already comes to UT for daycare, this form is not necessary.

Loft Boarding Contract (Dogs - Loft Boarding)
This is the agreement on drop off times, pick up times, rates, etcetera for dogs coming for either overnight or day boarding in our Loft Suites.  It is not needed if you are an existing Daycare customer.

Loft Pooch Daycare Addendum (Luxury Lofts)
If  you're dog is/has been staying in our Lofts and you would like to have them Temperament Tested to see if they can play with the other dogs while they are here, please fill out this form.  It gives us the extra information about your dog's social skills that was not included in your Loft Pooch Personality Profile.

 

Client Acknowledgement (Overnight Stays)
This Acknowledgment is to ensure that we are all on the same page about some of our critical policies such as deposits, refunds, etcetera.  This needs to be completed and on file prior to your pet's first overnight stay.

Save time at check-in by bringing in completed Check-In Forms for overnight stays:

1.  Sleepover & Loft Boarding Check-In (Dogs & Cats - Anyone spending the night)
This tells us where you are on this trip, when you'll be back, how much to feed your pet, needed medicines, etcetera.  Very important form!  Needed for any dog or cat spending the night whether it's in the Loft Suites, Cat Suites or with the other pups.

2.  Healthy Pet Guarantee (Dogs & Cats - anyone spending the night)
Our Healthy Pet Guarantee gives you peace of mind that if your pet gets ill while staying with us, or within 7 days after the stay, Urban Tails will pick up the tab.  You'll understand that we except from this any pre-existing conditions that your pet has.  See our rate page for the cost to add the Healthy Pet Guarantee to your pet's stay and the limit on coverage.  We ask that you Accept or Decline this coverage with each stay.  Claims do need to be submitted to Urban Tails within ten days of the pet's departure and include copies of the veterinarian's invoice and diagnosis.

Forms may be completed online. Unfortunately as of yet we still are not modern enough for you to submit them online, that's coming soon (we hope).  After completing the forms, please print them and either drop-off, mail or fax them to the below address/fax number.
